Please remember that it is important to sign pages 2 and 3 of the CIR doc 4 times as indicated.
DO NOT worry about the areas that have been left blank in the samples. You can leave them blank, strike through or simply put N/A
When you send them back in the provided email, you must provide the required ID also.
Okay good luck to all those who are still with PSX, looks like they are still moving forward. Quite optimistic but my guess is this will still take a few months more to be stable after that if payout is great good for us!
